FILING LOCALLY?
PREPARE YOUR DA 1559
BEFORE VISITING THE IG

•  Please note: Starting with your unit or base Inspector General is the fastest and most effective method of filing a request for assistance. 
•  Download a DA Form 1559 to your computer. (You may need to download it and open it on your device in order to see it in a PDF application)
•  Fill out the form digitally on your device OR print it out and complete it by hand. Be as accurate and complete as possible. 
•  Gather any supporting documentation you need for your case. 
•  Bring the completed form and documentation to your unit's Inspector General, or email it if possible.
•  If you are unable to download or print the form, your unit's Inspector General will assist you.
